Chicago, Illinois 60646
Telephone: 773-202-4130

This center offers a specialized adult day program for men and women diagnosed with Alzheimer's disease or related dementias, such as small strokes or Parkinson's disease. By providing up to 6 different program groups, the staff is able to support the well being and comfort of individuals with different needs and abilities. This is accomplished by matching them with others in groups of those with similar needs and abilities. Men and women can be cared for in this program from the earliest stages to the most advanced phases of dementia. Within each group a range of meaningful activities are designed around the person's capabilities and their interests. There is a primary focus on person-centered care, addressing the full range of needs that all individuals share, while honoring the individual's unique characteristics. Every effort is made to assure that humane and compassionate connections can flourish despite a decline in some functions.

For further information, simply call the above number. Any staff member who takes your call can be of immediate help by answering your questions, scheduling a tour or assisting you in any possible way.

United Methodist Homes & Services is a non-profit, non-sectarian organization that has been providing programs and services for older adults for over 100 years. Our continuum of care includes The Methodist Home, Hartwell House Assisted Living, Low Income Senior Apartments, United Methodist Community Support Services and White Crane Adult Day Wellness Program.

United Methodist Community Support Services is a paraprofessional home care program that enables older adults to remain safe and supported within the comforts of their own home. Our services include light housekeeping, laundry, personal care/hygiene, meal preparation, medication reminders, grocery shopping, transportation, escorting to appointments, companionship, respite care and over night/live-in care. We apply a personal approach to our program which includes:

  • Visiting with the person that will be receiving the care and the family or social worker involved before the service begins.
  • Taking the caregiver to the person's home prior to the service beginning.
  • Spending time with the caregiver to know them and schedule them appropriately with the client.
  • United Methodist Community Support Services is responsible for managing, monitoring and paying the caregiver.

In order to better meet the needs of older adults in our neighborhood, we work together with many local organizations such as Christian Churches Caring, A Helping Hand for the Elderly, Inc., Partner's Home Care, parish nurses and local hospitals. If you would like additional literature about our programs please call Tricia at 773-769-5270. I am also available to give presentations at churches or community groups to inform people on what to look for when they require services for themselves or older family members. We welcome all inquiries.
